RESPONSIBILITIES

You will assist the Medical Social Worker in assessing, evaluating, planning, coordinating resources and services for the continuity of care for the residents & collaborate with multidisciplinary health service professionals to develop care programs for timely treatment or recovery.

Monitoring of Bed Occupancy Rate and meeting admission KPIs on a monthly basis
Keeping track and managing the residents' data and information
Ensuring proper filing of residents' data and information
Applying financial schemes and monitoring the payouts for residents on a monthly basis
Monitoring and ensuring that residents' Household Means-Test is renewed before expiration
Preparing Medifund reports and documents to present to Medifund Committee
Answering and addressing queries and concerns from residents and their next-of-kin
Any other case management work as assigned

QUALIFICATIONS

Diploma in Nursing, Social Work or other related disciplines
Minimum 1 year experience in community healthcare settings in Singapore
Good knowledge of the geriatrics and community services
Strong communication skills (oral and written)
Proficiency in G Suite applications (e.g. Google mail, docs, sheets, slides, calendar)
