1. Casework & Counselling
- Conduct comprehensive psychosocial assessments for current and new admissions
- Develop, implement, and review individualised care plans that remain responsive to evolving needs
- Provide case management and care coordination for residents and their families
- Deliver emotional support and therapeutic counselling to residents and caregivers
- Support families through transitions, including long-term care planning, and end-of-life issues
2. Supervision & Mentorship
- Provide clinical supervision, guidance, and case consultation to Social Workers and Social Work Associates
- Model best practices in ethical decision-making, case formulation, and client engagement
- Contribute to the professional development of the team through coaching, training, and reflective practice
3. Interdisciplinary Collaboration
- Participate in multidisciplinary team meetings to ensure holistic, person-centred care for residents
- Work closely with nursing, allied health, and medical colleagues in care planning and implementation
- Build and maintain strong working relationships with external agencies, including hospitals, government bodies, community organisations, and social agencies to mobilise resources for residents
4. Professional and Ethical Standards
- Uphold the Singapore Association of Social Workers’ Code of Ethics in all aspects of practice
- Ensure compliance with documentation, audit, and reporting requirements (e.g. MOH, AIC)
- Stay updated on current best practices and contribute to the refinement of care models within the Lodge
