Now Hiring: Temporary Care Pal (Patient Outreach & Support)

Location: Pasir Ris - Polyclinic

Salary:

• $10.50/hour (for N/O/NITEC/A-Level holders)
• $13.00/hour (for Local Diploma holders)

Working Hours:

• Monday to Friday only
• Regular Shift: 8:00 AM – 5:00 PM (1-hour lunch break)
• Occasional Early Start: 7:00 AM – 4:00 PM or 7:30 AM – 4:30 PM
• No ad-hoc Saturday mornings required at this time
• Commitment required through end of November 2025 (with possible monthly extensions)

Job Responsibilities:

• Make appointment reminder calls and assist with scheduling
• Conduct patient outreach through SMS, calls, and mailers for health enrolments
• Process backend enrolments referred by doctors, healthcare teams, and home providers
• Assist patients in signing up for health support programs and coordinating consultations
• Support public education efforts and communicate health program details
• Follow up with patients on health plan completion
• Update and manage patient data to enhance care coordination

Vaccination Requirement:

• Mandatory medical screening (Tdap, MMR, COVID vaccines)
• Vaccination cost is covered if candidate commences work
